Who is eligible for assistance?
     
     Any patient/family who is receiving active treatment for cancer or cancer related illness, that 
     is  a resident of Medina, Holmes, Ashland, Wayne, Summit or  Stark County;   
     
     If you or your immediate family member are in need of financial assistance that cannot be 
     covered by insurance or other means of support;

     If you can show need of assistance by providing verification and documentation;


Request can be made for: (Payment is made directly to the provider and not the patient or their family)
     
         
     Utility bills
     
     Mortgage or rent payments


How do I apply for assistance?

     Complete the Grant Application Form with help of your social worker and mail it to the 
     Mary Grace Memorial Foundation.


     The foundation board will review your completed application and contact you regarding your 
      request.

    
     You may submit only one application per year.
