Who is eligible for assistance? 

• Any patient/family who is receiving active treatment for cancer or cancer related illness, that is a resident of Medina or Stark County.
• If you or your immediate family member are in need of financial assistance that cannot be covered by insurance or other means of support;
• If you can show need of assistance by providing verification and documentation;

Request can be made for: (Payment is made directly to the provider and not the patient or their family)

• Utility bills
• Mortgage or rent payments

How do I apply for assistance? 

• Complete the Grant Application Form with help of your social worker and mail it to the Mary Grace Memorial Foundation.
• The foundation board will review your completed application and contact you regarding your request.
• You may submit only one application per year.
